Physical Therapist (Per Diem, Days)

NorthBay Health Fairfield, CA (Onsite) Full-Time

At NorthBay Health the Physical Therapist works under the supervision of the Rehabilitation Supervisor and Rehabilitation Services Manager. The staff physical therapist, in accordance with his/her evaluation findings and the physician’s referral, plans and implements specific treatment programs for individual patients according to the principles and practices of physical therapy. Collaborates with the interdisciplinary treatment team. 

PRIMARY JOB DUTIES

  1. Provides physical therapy services in inpatient and outpatient settings during assigned shift during department operating hours, including assignment in rotation of weekend and holiday coverage.

  2. With physician’s referral, evaluates patients, develops specific treatment plans and goals, administers procedures and modalities specific to the learning needs and age of the patient, and plans for discharge utilizing appropriate Physical Therapy techniques.   

  3. Acts as a Rehab case manager for assigned patients.        

  4. Prepares and maintains accurate and current patient records in accordance with departmental policies and procedures. 

  5. Re-evaluates and revises patient’s treatment plan on a regular basis, with approval of physician as necessary.

  6. Provides patient, family/caregivers with age appropriate instruction regarding therapy procedures and home program, and provides additional instruction as indicated.

  7. Instructs the patient and family/caregiver in care and use of assistive and adaptive equipment. 

  8. Communicates with rehabilitation team members, family, physicians and third party payers regarding patient progress and program changes as appropriate.

  9. Attends and/or participates in inter/intradepartmental in-services and meetings as required and appropriate. 

  10. Participates in short and long-term program development, including participation on committees as requested.

  11. Participates in student intern and/or volunteer program as appropriate.

  12. Provides supervision to physical therapist assistant. 

  13. Maintains clean, safe and orderly treatment areas and equipment.  Reports to supervisor any problems or suggestions regarding safety in the workplace. 

  14. Assists with orientation and supervision of other health care professionals as appropriate.

  15. Establishes and maintains effective communication with physicians, co-workers, patients and other hospital personnel.

  16. Enhances professional growth and development through participation in educational programs, in-service meetings, and workshops within assigned specialty areas.

  17. Performs other duties as assigned.

  18. If working in Neonatal Intensive Care Unit, all notes in medical record must be co-signed by a CCS paneled physical therapist. 

  1. Education:  Minimum bachelor’s degree in Physical Therapy or equivalent.

  2. Licensure/Certifications: Current California license in Physical Therapy is required.  Physical Therapy License Applicant may be considered. Current AHA or equivalent BLS certification and all other annual requirements of System and department. (TB, Safety, etc.) If working in Neonatal Intensive Care Unit, CCS paneling is preferred and must be applied for once eligible. 

  3. Experience: One year of experience in acute hospital or outpatient clinic preferred, however, will consider new graduates.

  4. Skills:  Good basic clinical skills in physical therapy evaluation and treatment for a wide range of physical disabilities. Must demonstrate and maintain current knowledge and skill in providing appropriate care for patients in the following age groups: Pediatric, Adolescent, Adult, and Geriatric. Knowledge of existing laws and regulations governing the practice of Physical Therapy.   

  5. Standards of Performance: Demonstrate performance by adhering to established policies and procedures and exhibiting the defined characteristics associated with attendance and punctuality.

  6. Physical Effort: Attendance is an essential function of the job. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  This may include moderate lifting/pushing/pulling of patients and equipment in the inpatient and outpatient settings.

  7. Hours of Work: Assigned shift during department operating hours, including assignment in rotation of weekend and holiday coverage (required).

Compensation Structure

NorthBay Health uses a structured compensation framework. Staff-level positions use a step-based system (Steps 1–5) based on years of directly related experience, with Step 5 representing 20 or more years of experience in the role. Manager level and above positions are paid a fixed annual base salary and are eligible for a variable incentive compensation plan. Physician compensation is structured based on specialty and role requirements.
