Hospice Chaplain Bereavement Coordinator (part-time, days, FF)
JOB SUMMARY
At NorthBay Healthcare the Hospice Chaplain/Bereavement/Volunteer Coordinator Works under the supervision of the Manager of Hospice. Provides spiritual care to patients and families and caregivers in the NorthBay Hospice program in a timely manner. Participates in Hospice IDT to meet identified spiritual needs. Provides spiritual support to staff. Serves as liaison and support to community chaplains. Documents all visits and notifies case manager or director of any concerns/needs. Communicates visit to staff via secure message. Provides for funeral or memorial services for patients as requested. Accepts and performs other related duties and responsibilities as required.
Directs volunteer and bereavement services in the home and institutional setting. Directs volunteer services which provide assistance to patients, family, bereavement survivors, and bereavement group members through visits, phone calls, and support groups in accordance with the plan of care. The bereavement/volunteer coordinator also is responsible for the direction of bereavement services with the collaboration of the hospice team members. The bereavement coordinator conducts bereavement assessments, coordinates and provides family and individual counseling if indicated and makes bereavement visits. Coordinates internal and external educational offerings; and referral(s) to external resources if indicated
JOB SPECIFICATIONS
- Education: Graduate of an accredited seminary or school of theology. One year of experience working with Hospice or volunteer group. Bereavement facilitator certification or equivalent.
- Continuing Education Requirements: Hospice personnel are expected to participate in the appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, Hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs. Computer skills required. Demonstrated experience in working with employees, volunteers and ideally Hospice team member. Demonstrated experience in the development of programs, or work related to employee management.
- Licensure/Certifications: B.C.C. or M.Div. or equivalent theological degree. Current California drivers’ license. Access to operating automobile with car insurance that meets agency standards
- Experience: Experience in working with persons engaged in traumatic life experience in a health or hospice-related setting, or six months previous work-related experience with grief and loss counseling in the health care setting. Hospice/home health experience preferred.
- Skills: Computer literate with word processing and spreadsheet experience. Ability to work as a team member, respect for others and commitment to confidentiality. Good human relations skills, capable of communicating with others in a kind, courteous and professional manner and integrity and flexibility in meeting patient/family needs.

Compensation:
Hourly Salary Range Min $37.11 - Max $40.90 (Offered hourly rate based on years of experience)
